It used to be, here in the US. I had a tubal in 1983, after my second child. My husband had to sign a consent. Women are not yet free/equal. Progress, yes. Still work to be done!
@CarolinaGirl60 my aunt, at 45 years old, had to have her husband sign off for her hysterectomy. I've been, for 10 years, trying to get them to sterilize me and take out my ovary. I've been told no every single time. Nevermind that I have genetic and physical disabilities by the dozens and my right ovary looks like exploded popcorn. It's been a problem since I was ten years old. I had always been told "no no you'll change your mind you have to have at least 3 kids and your husband's permission blah blah blah"
