Hell is abuse, the others are ignorance...

Are we doing it?

Is it a cycle?

It's happening. Get on it.

I don't think these things can be classified as child abuse. If people actually believe these things, they are teaching their children what they "know" to be true.

When it comes to classifying abuse, I believe intentions matter. All of these things on the list are due to ignorance. Not vaccinating your children, for example, could be considered negligent, but if parents truly believe they are protecting their children by not vaccinating them, that's not child abuse. Even though they are putting their children at great risk, it can't be considered the same as beating them or locking them in a shed.
