Time to get a different doctor and maybe a lawyer too!!

Fine if the doctor asks whether you have discussed it with your husband/wife, and also checks your mental ability to make such a decision, that is his social responsibility.
However, to demand a signed form, other than as an indemnity, is not acceptable.

I had mine done in 1986, & the only permission I needed was mine!

I definitely think it should be each individual's right to make the decision about whether or not to be sterilized. Interesting there are 3 States that require the spouse's permisson

I signed written permission forms for both of my husband's to get vasectomies. Of course that was many years ago.
