In this context I do not think that there needs to be another category no, certainly not for any practical reason. But I understand that some agnostics and atheists are sensitive about their ideas being classified as a religion, because under some circumstances that is used by theists, as a lead in to the argument they use, that, there is no such thing as not believing.
Was this at a hospital that has a religious affiliation? If not, it is a strange question to ask. I suppose they have good intentions. I would probably respond "none".
No religious affiliation at all. I think lumping atheists into religion shows a lack of understanding about what atheist means.
It depends. If by atheist one refers to someone who does not believe there is a god, then no, that is not a "religion".
But if the term refers to someone who believes there is no god, then yes, because that is a belief system concerning the existence of a god without any evidence to support that belief.
Just as there is no evidence that a god exists, there is no evidence that a god does not exist.
