"When questioned by the ABC this morning, NSW Premier Gladys Berejiklian said she wasn't aware of the incident but said "mistakes are going to happen".

How long are we going to give authorities a free ride? Sure mistakes happen, but so does cost cutting, inadequate training and incompetent management. Second time no longer a mistake imho. Not good enough

What's new in NSW? It's been happening for as long as I've been in Australia.

Refresh your memory onwhy canberra amongst many places had bad fires a few years ago, Frenchmen shot on the beach and one of my early memories was the deaths fom the Granville bridge & train disaster.

Bushfires happen in this dry environment of Australia and happened since time immemorial.
Your beloved Victoria is DEFINITELY NOT the pristine, perfect State you think it is.
Rail disasters, even the horrific Granville one, happen, funny but I seem to remember a few nasty ones happening in the last 12 months to 2 years in Victoria, or don't they count?
How many Miners were shot and killed Eureka and the other goldfields in Victoria by your wonderful, corrupt Victorian Police, aka the TRAPS. Btw at least 90% of your Victorian Police at the times of the Goldfields, etc, WERE of Irish decent and WERE PREYING on their OWN peoples as well as numerous others as well.
